Medical Records

PLEASE NOTE:

We are temporarily restricting access for in-person Medical Record requests. If you are in need of your medical record please call 617-499-5027

We will provide you a copy of your medical record via MyChart or mail

A portion of your medical record can be found in your under the Health tab.

If you would like to have a copy of an electronic record not found in MyChart, complete the Request Medical Records form located under the Health tab in your MyChart account. You may also call the Health Information Management Receptionist at 617-499-5665, ext. 3049, Monday through Friday, 9 am to 4 pm (excluding holidays). If you wish to obtain a paper copy you will need to complete and sign an authorization form. This form also allows you to send and receive your medical information.

Fees

There is no charge to send reports directly to another hospital, nursing home or physician.

For personal copies of your medical record:
Pursuant to HIPAA 45 CFR, 164.524, we reserve the right to charge a reasonable cost-based fee for producing and mailing the copies. If you want the entire medical record or more than the standard two-year abstract (includes discharge summaries, history of illness, test results, medications, operative notes, hospital courses, discharge diagnoses and 5 years of radiology reports) the rate will increase proportionately based on the cost. It is rarely necessary for a page by page copy of your entire medical record.

Xổ số may mắnFor all other release of information requests, the applicable US state statute governing fees for medical records will be applied.

Privacy

Your privacy is our priority. The Privacy Notice, elements of which are mandated by federal law, is lengthy and may be somewhat confusing. Federal law has changed little in the practice of the release of Protected Health Information (PHI), and the laws of Massachusetts governing PHI are stricter than federal law. An authorization to release PHI must be signed by the patient, or the patient's legal representative (guardian or person with durable power of attorney). If you have questions about patient privacy, please contact Mount Auburn's Privacy Officer at 617-441-1665.

Birth and Death Certificates

Mount Auburn Hospital does not maintain copies of birth certificates or death certificates. To obtain these, please contact Cambridge City Hall or the Massachusetts Department of Vital Statistics.
